How Can Your Staff Team Learn from Mistakes?

In health and social care mistakes can feel heavy. Helping individuals and teams grow stronger, wiser and more confident after mistakes have happened is key.

If people are afraid to speak up, they won’t. That’s when small mistakes can be hidden and turn into bigger ones. Ensure your staff team knows:

  • It’s okay to get things wrong
  • It’s expected to make mistakes, it’s how we learn
  • It’s safe to ask for help or clarification

Focus on systems, not individuals

Ask yourself: Was it really a one off? Or did our process set them up to fail?

  • Were the instructions clear?
  • Was the person trained and supported?
  • Did we spot a pattern too late?

After an incident (even a minor one), use it as a learning opportunity:

  • What happened?
  • What could we do differently?
  • What do we need to improve or adjust?

This could be part of a team meeting, a supervision, or an informal debrief.

 

Protect Dignity and Accountability

Support doesn’t mean a lack of responsibility. Clear boundaries and respectful feedback are essential for building a safe, accountable team.
