Aim: To develop a clear understanding of care plans. Objectives: By successfully completing this course you will be able to:

• Describe what are care plans and why are they important
• List the relevant legislations in the UK
• Describe why it is important to ensure the person receives the same care regardless of which care worker is on duty
• Understand the importance of recording the care which is being provided
• Understand what is meant by person-centred care planning and how to support the individual in identifying and managing their own care needs
• Understand how to write effective care plans
• Describe what care plan reviews and working documents are
• Describe how to develop advance care plans

Course Duration: Face-2-Face session will be delivered at your location for up to 16 attendees. Cost of this course is for all 16 attendees. eLearning: 3 hours.
